LPN

Responsibilities:

S
ummary/Objective:
The LPN is responsible for monitoring the health care needs of individuals served in the Intermediate Care Facility for Persons with Intellectual Disabilities (ICF/ID). The LPN will perform nursing care and programming of individuals accordingto established rules and regulations and in accordance with standards of nursing practice.

E
ssential Job Functions:

  • Report issues of concern to ICF/ID Director and/or ICF/ID RN as appropriate. Offer ideas and suggestions forresolution of concerns. Use appropriate methods and channels of communication.
  • Comply with all applicable policies, practices, and laws, and with the regulations provided by any governing bodies, such as Department of Human Services (DHS) and Department of Inspections and Appeals(DIA).
  • Coordinate Health Care:

oReferral of consumers with health problems to an appropriate physician/medical professional.

oSchedule routine medical appointments, to include, but not limited to physical, dental,

optometry, audiology and laboratory appointments. Notify needed individuals of appointments

and coordinate transportation.

oEnsure all medications, tube feedings and treatments are administeredto consumers in compliance with physician's orders.

oMonitor infection control practices in the home setting.

oIn coordination with QIDP and Psychiatrist, establish drug reduction plans for appropriate

consumers.

oPossess a strong knowledge base of psychotropicmedications used for behavior management, indication for use, side effects, etc.

oReview seizurereports and compile monthly seizurereports.

oMaintain current immunization status for consumers.

oPossess knowledge of consumers programs and procedures and implement as written.

oMaintain communication with family/guardian, program coordinators and operation

supervisors regarding health concerns.

  • Training Individuals Served:

oTrain consumersto self-administer medications as appropriate to their developmental levels and approved by the IPP team.

oTraining consumer's appropriate health and hygiene methods and infection control practices, as appropriate to their learning ability.

oTraining consumers in detecting and reporting symptoms of illness or dysfunction, as appropriateto their learning ability.

  • Assessment Responsibilities:

oImplement, update and discontinue Skin Integrity Flow sheets.

oAssess consumers health care needs, plan and implement appropriate nursing interventions

and evaluate outcomes.

oComplete quarterly psychotropicmedication reviews for appropriate consumers.

oComplete follow up assessments on consumer incident reports.

oComplete Abnormal Involuntary Movement Assessments on appropriate consumers.

oParticipate in the development, review, and update of an individual program plan as part of the

interdisciplinary process.

oComplete quarterly assessments of consumers, including a full visual examination and review of all body systems.

  • General Responsibilities:

oReview incident/accident reports and behavioral incident reports of persons served and document appropriate follow-up. Seek out the root cause of incidents and provide follow-up to decrease the likelihood of reoccurrence.

oProvide for the safety of the individuals served with the least amount of intrusion as possible.

oPerform direct care duties as needed, including assistance with bathing, toileting, meals, repositioning, lifting, etc.

oMaintain a clean work area, including regular cleaning of cupboards, cardexes, medication refrigerators, work counters, office space and medical equipment.

  • May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
  • May require prolonged sitting and working at a computer for up to eight hours.
  • May require frequent bending, stooping, and stretching.
  • May require prolonged standingfor up to eight hours.
  • Must wear hearing and eye protection in required areas.
  • Must have ability to see objects/persons at a distance.
  • Must have ability to see close work such as typed or handwritten material.
  • Must have ability to hear conversations in a quiet and a noisy environment.
  • Must have ability to determine where a sound is comingfrom.
  • Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
  • Must have ability to communicatethrough speech.
  • Must remain awake during working hours, if applicable.

referred Education and Experience:

  • At least 18 years of age.
  • High school diploma or GED.
  • Current nursing licensure and in good standing in the state of Iowa.
  • Experience with persons with disabilities required.
  • Supervisory and leadership skills/experience required.
  • Basic computer skills including MS Word.
  • Background checks with previous employers, references, law enforcement agencies, and others will be conducted and found satisfactory.
  • Valid driver's license and driver's record deemed acceptable by Imagine the Possibilities, Inc.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
