Summary/Objective: The LPN is responsible for monitoring the health care needs of individuals served in the Intermediate Care Facility for Persons with Intellectual Disabilities (ICF/ID). The LPN will perform nursing care and programming of individuals according to established rules and regulations and in accordance with standards of nursing practice.
Essential Job Functions:
Report issues of concern to ICF/ID Director and/or ICF/ID RN as appropriate. Offer ideas and suggestions for resolution of concerns. Use appropriate methods and channels of communication.
Comply with all applicable policies, practices, and laws, and with the regulations provided by any governing bodies, such as Department of Human Services (DHS) and Department of Inspections and Appeals (DIA).
Coordinate Health Care:
o Referral of consumers with health problems to an appropriate physician/medical professional.
o Schedule routine medical appointments, to include, but not limited to physical, dental,
optometry, audiology and laboratory appointments. Notify needed individuals of appointments
and coordinate transportation.
o Ensure all medications, tube feedings and treatments are administered to consumers in compliance with physician's orders.
o Monitor infection control practices in the home setting.
o In coordination with QIDP and Psychiatrist, establish drug reduction plans for appropriate
o Possess a strong knowledge base of psychotropic medications used for behavior management, indication for use, side effects, etc.
o Review seizure reports and compile monthly seizure reports.
o Maintain current immunization status for consumers.
o Possess knowledge of consumers programs and procedures and implement as written.
o Maintain communication with family/guardian, program coordinators and operation
supervisors regarding health concerns.
Training Individuals Served:
o Train consumers to self-administer medications as appropriate to their developmental levels and approved by the IPP team.
o Training consumer's appropriate health and hygiene methods and infection control practices, as appropriate to their learning ability.
o Training consumers in detecting and reporting symptoms of illness or dysfunction, as appropriate to their learning ability.
Assessment Responsibilities:
o Implement, update and discontinue Skin Integrity Flow sheets.
o Assess consumers health care needs, plan and implement appropriate nursing interventions
and evaluate outcomes.
o Complete quarterly psychotropic medication reviews for appropriate consumers.
o Complete follow up assessments on consumer incident reports.
o Complete Abnormal Involuntary Movement Assessments on appropriate consumers.
o Participate in the development, review, and update of an individual program plan as part of the
interdisciplinary process.
o Complete quarterly assessments of consumers, including a full visual examination and review of all body systems.
General Responsibilities:
o Review incident/accident reports and behavioral incident reports of persons served and document appropriate follow-up. Seek out the root cause of incidents and provide follow-up to decrease the likelihood of reoccurrence.
o Provide for the safety of the individuals served with the least amount of intrusion as possible.
o Perform direct care duties as needed, including assistance with bathing, toileting, meals, repositioning, lifting, etc.
o Maintain a clean work area, including regular cleaning of cupboards, cardexes, medication refrigerators, work counters, office space and medical equipment.

May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two-person lift, mechanical lift, or assistive equipment.
May require prolonged sitting and working at a computer for up to eight hours.
May require frequent bending, stooping, and stretching.
May require prolonged standing for up to eight hours.
Must wear hearing and eye protection in required areas.
Must have ability to see objects/persons at a distance.
Must have ability to see close work such as typed or handwritten material.
Must have ability to hear conversations in a quiet and a noisy environment.
Must have ability to determine where a sound is coming from.
Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
Must have ability to communicate through speech.
Must remain awake during working hours, if applicable.
Travel: Limited travel is expected for this position.
Preferred Education and Experience:
At least 18 years of age.
High school diploma or GED.
Current nursing licensure and in good standing in the state of Iowa.
Experience with persons with disabilities required.
Supervisory and leadership skills/experience required.
Basic computer skills including MS Word.
Background checks with previous employers, references, law enforcement agencies, and others will be conducted and found satisfactory.
Valid driver's license and driver's record deemed acceptable by Imagine the Possibilities, Inc.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
